Pesticide Use Reporting (PUR) Forms

The Pesticide Use Reporting (PUR) forms are available in two (2) formats:
PDF--You can download and save the PDF versions to your local system or you can fill it out on-line. In both cases, you will print it from your local printer. Remember, when using the PDF version, if you are using the free Acrobat Reader, you cannot save the data that you write into the form. If you need to make a change after printing and exiting the form, you can either reenter all of the data or hand write the updates on the printed form (If you have purchased the full version of Adobe Acrobat, you can save PDFs along with any data entered.)
FML(Omniform)--You can download and save the Omniform versions to your local system. You can also fill the form out and save the data to your local system. This means that you can make changes to existing data, as well as update (add to) the form with new data during the reporting period. You will need to download the Omniform Internet Filler.
(The Omniform Internet Filler is no longer available free of charge; but if you have already downloaded the filler, you can still use the fml version of the forms.)
